- Parental consent must be obtained before medication
can be given to a student. Only the School Nurse, Lisa Pinsky, may administer the medications in accordance
to the established protocols.
- The Emergency Card must be filled out and signed by
a Parent/Guardian.
- This form must be completed by a Physician, Nurse
Practitioner or Physician's Assistant prior to a student
participating in any of the after-school athletics
programs.
- This form must be completed by a parent or legal guardian
or the student if over the age of 18 before a students'
records can be released.
